Note:  The form refers to the UW-Fox Valley "Hold Harmless Agreement," which you can read here.
A responsible adult must sign the registration form before camp begins.

Refund policy  After payment is received: $25 fee to cancel; within two weeks of session, 50% of registration costs will be refunded with cancellation; no refunds once session starts.   Registrations will be acknowledged in writing upon receipt, and confirmation letters will be sent out for each session as minimum enrollments are met.
